ABSTRACT

Objective To investigate the correlation between prognosis of patients with severe acute pancreatitis and changes of plasma albumin. Methods A total of 235 patients with severe pancreatitis who were admitted into hospital from September 2008 to October 2013 were selected as research object. According to the prognosis, they were divided into the survival group (143 cases who were recovered and still alive) and the death group (92 cases who died of the deterioration of diseases). Retrospectively analyzed the basic situation of the pa-tients and the changes of serum albumin levels in the two groups. And the correlation between prognosis of patients with severe acute pancrea-titis and changes of plasma albumin were evaluated by the logistic regression analysis. Results Compared with the death group,there were significant differences (P<0. 05) in terms of age,APCHEⅡ score,Ranson score,SOFA score,length of hospital stay,duration of mechani-cal ventilation,continuous renal replacement therapy,average serum albumin level,decrease of serum albumin within 24 hours,and variation of serum albumin. And ROC curves suggested the best cutoff value of decrease of serum albumin within 24 hours to predict death in patients within 24 hours was 4. 24 g/L,and the best cutoff value of variation of serum albumin to predict death was 13. 6%. Logistic regression analy-sis showed that the major risk factors of death in severe acute pancreatitis were decrease of serum albumin within 24 hours,variation of serum albumin within 24 hours,average serum albumin level,and the APCHEⅡscore. Conclusion Changes in plasma albumin can reflect the se-verity of severe acute pancreatitis,and the sharp decrease of serum albumin within 24 hours can be used as a risk factor for death.

ABSTRACT

@#ObjectiveTo investigate the correlation of lipids and lipoproteins with cardiac function in patients with coronary heart disease (CHD).MethodsParameters of blood pressure, serum lipids and lipoproteins, clinical features, radionuclide ventriculographic and coronary angiographic findings were analyzed retrospectively in 584 CHD patients treated with standard treatment.ResultsThe left cardiac dysfunction of CHD patients was correlative positively with age, systolic blood pressure (SBP), diastolic blood pressure (DBP), blood glucose, body mass index (BMI), smoking, total cholesterol(TC) and low density lipoprotein cholesterol(LDL-ch) significantly(P<0.001), not correlative with high density lipoprotein-cholesterol (HDL-ch). The left ventricular ejection fraction (LVEF) and left ventricular peak filling rate (LVPFR) were decreased along with the levels of TC and LDL-ch increased (P<0.05~0.01).ConclusionTC and LDL-ch are positively correlative with left cardiac dysfunction in CHD patients. In order to reduce cardiac damage of CHD associated with hyperlipoidemia, more attention should be paid to the amelioration of coronary arteriosclerosis and the reduction of serum lipids and lipoproteins.

ABSTRACT

Objective To study the diagnosis,management and prognosis of persistent m?llerian duct syndrome (PMDS). Methods 2 cases of PMDS associated with transverse testicular ectopia were reported.Clinically there was azoospermia or testis tumor.Corporeal hysterectomy and orchidopexy were performed and the testis tumor was excised. Results Vascular supply and hormonal function of the testis were normal in both 2 patients after 2 year's follow up. Conclusions PMDS is an inherited male pseudohermaphroditism. Every effort should be made to preserve the testis and its fertility function when surgical management is needed.The possible development of testis tumor should be kept in mind and closely observed.

ABSTRACT

AIM: To elucidate the in vivo mechanisms of the proliferation of vascular smooth muscle cells (VSMCS) in injuried arteries. METHODS: A VSMCS proliferative model was constructed by injury of rabbit iliac arteries with balloon catheters and a probe designed for rabbit platelet-derived growth factor B chain (PDGF-B ) mRNA was used to detect the expression of it by intimal VSMCS on the vascular cross sections using an in situ hybridization technique at the indicated times. The relation of this expression to the proliferation of VSMCS by their expression of proliferating cell nuclear antigen (PCNA) and vascular intimal areas were estimated. RESULTS: The expression of PDGF-B mRNA of intimal VSMCS was increased when calculating the intimal PDGF-B mRNA positive cells per millimetre area at ?400 magnification with average numbers of 31.93?14.64 in 1 week group, 26 50?9 25 in 2 weeks group and 24 85?13 65 in 4 weeks group. This was in accordance with the expression of PCNA by VSMCS and the increase of intimal areas. CONCLUSION: The local production of PDGF-B by VSMCS via an autocrine mechanism is responsible for the continuous proliferation of these cells and formation of neointima after the injury. The probe designed is very useful for detecting rabbit PDGF-B mRNA.
